Preferred Disability Management, LLC (Hereafter noted as PDM) is a medical case management company that is routinely employed to assess and make recommendations relative to injuries/illnesses suffered by individuals who are either employed or insured by our clients. PDM routinely makes medical and vocational information available to our clients as consistent with client policy/requests and as permitted by statute. Said information has traditionally been provided via written report (mailed or faxed) or via electronic communication (e-mail). PDM is currently making this same information available through on-line access via a secure web site. PDM will allow access to this online records system by providing a password that can be used to access the appropriate company account. By clicking the “ACCEPT” button below, you are certifying the following:
  • You have a “need to know” regarding the information contained herein.
  • You will not share access to this account with anyone outside your company.
  • You will not share access to this account with any other company employees that do not have “need to know” status.
  • You agree to hold any information you access confidential according to applicable state and/or federal statute.
  • You agree that should your position change within your company, such that you no longer have a lawful need to access the information contained in these online records, you will notify PDM so that access passwords and/or codes can be changed as appropriate to protect the confidentiality of said records.
